Cancer screening

Prevention and screening are different

  • Prevention lowers the chance of cancer; screening looks for cancer or precancer before symptoms.
  • New symptoms need diagnostic assessment, even after a normal screening result.

Useful prevention steps

  • Tobacco and secondhand smoke: avoid exposure and seek support to quit.
  • Weight and movement: regular suitable activity and a healthy weight.
  • Food and alcohol: varied fiber-rich foods, less processed meat and avoidance of alcohol.
  • HPV and hepatitis B vaccination when appropriate for age and circumstances.
  • Sun and work: protect skin and reduce carcinogenic exposures.
HPV

Key tests for people without symptoms

  • Breast: one evidence-based average-risk schedule is mammography every 2 years at ages 40–74; high risk may require earlier screening and MRI.
  • Colorectal: ages 45–75; annual stool blood testing or, after a normal high-quality examination, colonoscopy usually every 10 years.
  • Cervix: one accepted schedule uses Pap testing every 3 years at ages 21–29; at 30–65, HPV testing every 5 years or Pap testing every 3 years.
  • Lung: annual low-dose CT for selected high-risk adults aged 50–80; smoking exposure and time since quitting must be assessed.
  • Prostate: PSA is not a routine test for everyone at every age; age, family history, benefits and harms inform a shared decision.
HPV
PSA

The plan is individual

  • The ages and intervals above are international average-risk examples; in Iran they need adaptation to health, history and available services.
  • Do not wait for screening when a new lump, unusual bleeding, unexplained weight loss or persistent symptom appears.

The plan is individual

  • The ages and intervals above are international average-risk examples; in Iran they need adaptation to health, history and available services.
  • Do not wait for screening when a new lump, unusual bleeding, unexplained weight loss or persistent symptom appears.

When does the schedule change?

  • Previous cancer, advanced polyps, abnormal results or inflammatory bowel disease can change testing intervals.
  • Cancer in several close relatives or at young ages may warrant genetic counseling and earlier screening.
  • Cervical screening start ages differ by guideline and test type; HPV vaccination does not remove the need for scheduled screening.
  • Beyond 75 for colorectal screening, and at older ages for other tests, health and prior results help determine continuation.

Important testing details

  • A positive stool test needs colonoscopy; a later negative stool test does not replace that follow-up.
  • Ultrasound alone is not the usual replacement for screening mammography; supplemental testing is individualized.
  • A commonly used lung criterion is at least 20 pack-years with current smoking or quitting within 15 years; some guidelines differ.
  • Twenty pack-years means, for example, one pack daily for 20 years; a person must also be able to undergo evaluation and treatment.

Benefits and limitations

  • Screening can detect cancer or precancer earlier and reduces cancer deaths for selected cancers.
  • Possible harms include false alarms, anxiety, extra procedures and detection of disease that might never cause harm.
  • Whole-body PET/CT or CT and panels of blood tumor markers are not recommended general check-ups for everyone.

Common questions

Is testing for symptoms still screening?

No. A new lump, unusual bleeding, blood in stool or a persistent cough needs diagnostic assessment. Do not wait for screening age or the next routine test. A previous normal result does not explain a new symptom.

Does a positive result mean cancer?

Usually not; many abnormal findings are benign. The next step may be additional imaging, colonoscopy or biopsy, depending on the test. Keep the report and complete follow-up until the finding is resolved.

Is an annual full panel better?

More tests do not necessarily provide more protection. Unnecessary testing can trigger false alarms and extra procedures. A written plan based on age, risk and previous results is more useful than the same check-up for everyone.

Does HPV vaccination replace screening?

No. Vaccination lowers the risk from important HPV types but does not eliminate every risk. People with a cervix should continue an appropriate screening schedule; abnormal results need specific follow-up.

How should I start my plan?

Bring your age, cancer and polyp history, smoking exposure, family history and prior reports. Record the test, next date and follow-up pathway. These international examples require adaptation to your circumstances.
